Forms and Surveys — StudioM Platform Guide¶
Who this guide is for: Anyone collecting leads, registrations, feedback, or applications on the StudioM platform. Time to read: Approximately 10 minutes
What Are Forms and Surveys?¶
Forms and surveys are how you collect information from people who visit your website, landing pages, or funnels.
- Forms are best for lead capture, registrations, applications, and contact requests — structured fields, one flow
- Surveys are best for gathering feedback, running assessments, or collecting opinions — question-by-question format with optional branching logic
Both automatically create or update a contact record when submitted, and both can trigger automated workflows.
Important: Once created, a form cannot be converted into a survey, and vice versa. If you need to switch types, you'll need to create a new one.
How to Get There¶
Navigate to: Sites → Forms or Sites → Surveys (left sidebar, under Sites)
You can also access forms from directly within a funnel or website page while building it — without leaving the editor.
Creating a Form¶
- Go to Sites → Forms
- Click Create Form
- Choose a starting point:
- Blank — build from scratch
- Template — start from a pre-built structure (contact form, application, registration, etc.)
- Use the form builder to add and arrange fields
- Customise the design (colours, fonts, button text)
- Set what happens after submission (redirect to a thank-you page, show a message, etc.)
- Click Save
Adding Fields¶
Drag fields from the left panel onto your form. Common field types:

Standard Fields vs. Custom Fields¶
- Standard fields (name, email, phone) map directly to contact record fields
- Custom fields you've set up in your account can also be added to forms — any data submitted populates the contact's custom field automatically
Form Settings¶
Click the Settings tab in the form builder to configure: - On Submit: What happens after someone submits — redirect to a URL, show a custom message, or go to the next page in a funnel - Sticky Contact: If enabled, the form pre-fills with known contact data if the visitor is already in your system - Facebook Pixel: Attach a pixel for conversion tracking (if applicable)
Creating a Survey¶
- Go to Sites → Surveys
- Click Create Survey
- Choose blank or a template
- Add questions one by one — each question is a separate "step" that the respondent sees on its own screen
- Add conditional logic if needed (e.g., if they answer "No" to question 3, skip to question 7)
- Set the completion action (redirect, message, enrol in workflow)
- Save
Conditional Logic (Survey Branching)¶
Surveys support skip logic — routing respondents to different questions based on their answers. This is useful for: - Qualification surveys (different paths for different customer types) - Feedback forms with follow-up questions based on their rating - Assessments that adapt based on responses
Adding a Form or Survey to a Page¶
Forms and surveys can be placed on any funnel page, website page, or landing page.
From the page builder: 1. Open the page in the builder 2. Drag a Form or Survey element onto the canvas 3. In the element settings, select the form/survey from your library 4. Adjust styling as needed 5. Save and publish the page
Sharing a standalone link: Every form and survey has its own direct URL that you can share by email, SMS, or social media — no page needed.
What Happens When Someone Submits¶
When a contact submits a form or survey:
- A new contact record is created (or an existing one is updated if the email/phone matches)
- Their submitted data populates the relevant fields
- Any workflow triggered by that form fires automatically
- You can see the submission in the contact's Activity timeline

Managing Your Forms and Surveys¶
From the Forms or Surveys list view, you can: - Edit any form or survey - Duplicate — create a copy to use as a starting point for a new one - View Submissions — see all responses and who submitted them - Share the direct link - Embed — get the embed code to place it on an external website
Viewing Submissions¶
- Open the Forms list
- Click on a form
- Click the Submissions tab
- See each submission with the contact name, date, and submitted data
- Click any row to see the full response
Collecting Payments on a Form¶
You can sell products or collect payment directly through a form by adding a Payment element.
- In the form builder, drag the Payment element onto the form
- Select the product(s) to sell
- Configure the order summary display
- Save

Best Practices¶
- Keep forms short — every extra field reduces completion rates. Only ask for what you actually need
- Use clear labels — field labels should be plain English, not jargon
- Always include email or phone — you need at least one to follow up
- Set up a workflow for every form — if someone submits and nothing happens, it's a missed opportunity
- Test your form before publishing — fill it out yourself and confirm the submission is recorded and any automation fires correctly
- Add a thank-you message — after submission, acknowledge that you received it and set expectations (e.g., "We'll be in touch within 24 hours")
Common Questions¶
Q: Can I embed a form on my own external website? Yes — every form has an embed code. Go to the form, click Share/Embed, and paste the code into your website.
Q: Will the form automatically create a contact? Yes — if the submitted email or phone matches an existing contact, their record is updated. If not, a new contact is created.
Q: Can I limit form submissions (e.g., close a form after 50 responses)? This requires a workflow with a condition check. Contact StudioM Support and we can set that up for you.
Q: Can I receive an email notification every time someone submits a form? Yes — set up a workflow with the Form Submitted trigger and add a "Send Internal Notification" action to email yourself or your team. See the Workflows guide for instructions.
Q: What's the difference between a Form and an Order Form? An order form is a form with a payment element — it collects contact details and payment in one step. A regular form collects information only.
